Colorado Choice Health
Plans’ history dates back to 1972, when it was established under the name San Luis Valley Health Maintenance Organization as one of the first health plans in Colorado. The name was changed in 2006 as they expanded their coverage area outside of the San Luis Valley in Colorado. The non-profit health plan now serves 30 counties, with their headquarters located in Alamosa, Colorado.
